Fix the wiimote speaker problem in the core and remove the HLE patch for it.

Real wiimote tested on windows, untested but should work on linux, dunno about osx.

Added an option to disable the Wiimote speaker. The checkbox has 3 states:
...
Ticked = Clear sound but a bit unresponsive to controls
Filled = Same as r7225
Clear = Disable speaker
The option is in the Wii tab of the configuration.

Added Wiimote speaker support by hack patching out the IsBusyStream function using HLE. DSP LLE is required to process the speaker data. This commit works with real Wiimotes. I have added the code to decode the audio from the emulated wiimotes but have not added the code to output their audio to the PC speakers.

* Added HLE function IsBusyStream which signals that the Wiimote is ready to accept speaker data. This function is not a conversion from the real PPC code. It a simple function that returns the "ready" status after being polled.
* Added code to find and patch HLE functions on boot
* Added 4bit Yamaha ADPCM decoder from the ffmpeg project
* Removed some test code
* Added some copyright notices
Fixes issue 438.

First pass at dealing with different size_t/off_t sizes in C90 environments.

Most of the code dealing with the LogTypes namespace was C which lead to a
lot of nonsensical casting, so I have dumbed LOG_TYPE and LOG_LEVEL down to
plain C even though the move of wiiuse into Source means we don't currently
call GenericLog from C.
Set logging threshold to MAX_LOGLEVEL at startup so debug builds will also
p rint debugging messages before the GUI is running.
For some reason the way we use SetDefaultStyle doesn't play nice with wx 2.9
so we just get the default black text on a black background. Using a gray
background works around that problem, but I found it to also be much easier
on the eyes so I have switched the background color on all versions.

New DSP debugger: step one. (not ready yet, but try loading zelda WW and look at the dsp debugger..).

Had to shuffle around quite a lot of code to be able to extract the CodeView into a library nicely so it can be used from both the main dolphin and the LLE plugin... also extracted the symboldb code.

PowerPC: Fixed moronic bug in mcrfs - together with correctly computing FPRF (which is a new option, slows down slightly) in a few ops this fixes most remaining math errors in Super Monkey Ball and eliminates the need for the HLE hacks. Make sure to have up-to-date gameconfigs. Unfortunately, F-Zero still doesn't work. Misc other small changes. Indent some code.

Fix DEBUG logging (didn't work!). Shuffle around the log levels to make more sense (now NOTICE is the top one and always on), the rest is ERROR, WARNING, INFO, DEBUG. Fix the log levels of a lot of stuff. Use macros instead of numbers for various log level checks.

an attempt to clean up the logging a bit

1) The new ERROR, WARN, INFO, DEBUG _LOG macros were used
ERROR, // Bad errors - that still don't deserve a PanicAlert.
WARNING, // Something is suspicious.
INFO, // General information.
DEBUG, // Strictly for detailed debugging - might make things slow.
2) Made all LOG macro use into some of the logging level supporting macros
LOG is commented out on linux can someone try it on windows? (it's in Log.h)
3) Added ERROR_lOG next to each panic alert
I hope I helped making the logs a bit more useful/readble
